The HEALTH QUESTIONNAIRE is required to be completed by all volunteers visiting Whenua Hou/Codfish Island OR Anchor Island.

Complete the questionnaire only when the Kakapo Team ask you to.  The Health Questionnare is required AFTER you have been provisionally placed in the roster, because the information disclosed must be up-to-date. 

When it is required, please download the questionnaire, complete, sign it and post it back to the address supplied.
